Converted MP3 files at 320 kbps are about 4× smaller, so far more music fits on the watch. Or keep the original lossless file, byte for byte. Cuff explains the tradeoff on first launch, and you can change it any time in Settings.

Every pair of headphones colours the sound a little. Cuff loads a measured correction profile for your model from the AutoEq database and applies it with a parametric EQ. Connect a different pair and Cuff switches to that pair’s profile.

No. Cuff runs on the watch by itself. You sign in, browse and download on the watch, and everything you download plays from the watch’s own storage. The only other device you need is a browser, once, to enter the link code at plex.tv/link.

For most people, MP3 320. Bluetooth headphones re-compress audio anyway, so MP3 320 sounds the same on a watch and saves lots of space: files are about 4× smaller, so far more music fits. Lossless is there if you want the exact file from your server, byte for byte. You can change your choice any time in Settings.
Only while downloading. Once music is on the watch it plays locally, so listening uses no bandwidth at all. Downloads run on Wi-Fi only by default. If you choose MP3, your server converts each track as it downloads.
With MP3 selected, your server converts whatever is in your library, so any format your server can play works. With lossless selected, Cuff keeps the original file and plays the common formats directly, including FLAC, MP3, AAC, Ogg Vorbis, Opus and WAV.
